Permission Forms
Young people are welcome to drop into most of our activities without a permission form. We require a form for trips, residential events, and some special activities.
Forms are valid for 1 year so if you have completed one within the last year you don't need to do one again for a while. (If you're unsure when you last completed a form just do one anyway or contact Gemma and ask whether or not you need to do one).
This form must only be completed by a parent or guardian.
You can complete one online, or print it out and bring it along to us.
